How do I request a Service?

• To request a service simply go to the Self-Serve Portal at http://services.diabetes.ca and click on the  "Request a Service" icon.

• On the Request a Service page select the appropriate form and click “Request Now”.
  
• Complete the request form and click “Place Request”
  
• The request will be assigned to a member of the IT team to provide the service.
  

NOTE: In some cases approval may be required before a request can be completed. You can track the progress of your request by clicking the "Your Tickets" icon in the Self-Serve Portal.
