IT & Facilities System Access Request Application


Introduction

The IT and Facilities System Access Request application is replacing the old Word forms for New Hires and Terminations.

It is easy and simple to use, and the same application can be used for New Hires or Terminations.

First Time Setup

If you are accessing this application for the first time you will be asked to grant access permission for it to use your Office 365 account. Click the Allow button.

Getting Started

 

 

To begin click on the Get Started button. This will take you to the next screen where you can pick the organization and the department you are working for

 

New Hire Request

 

 

 

Step 1: pick the organization, DC stands for Diabetes Canada or if you work for the National Diabetes Trust you want to choose NDT from the list.

Step 2: pick your department. There are different department names for DC and NDT.

Step 3: Click the Create New Employee Request button. This will ask you some more questions.

 

 

 

Choose Yes, if this employee is replacing a previous position holder in the same role. You will want to use this if for example you had a coordinator position open and you are filling it with the same role.

Select No if the New Hire is not replacing the same role.

 

 

 

 

1. Enter the Previous employee’s full name.

2. Next are a series of check boxes which you can click or leave blank depending on what access your new employee will need.

· Does the new hire use the same office space and phone?

· Does the new hire inherit the same system privileges?

· Does the new hire use the same computer?

· Transfer files from "My Documents" to new hire?

· Can new hire access emails and archives?

3. Click Next to go to your department’s form.

 

 

 

 

If you selected No in the previous screen because your new hire is not in the same role, the next two questions ask if you require a new phone, desk or computer.

If you already have everything, leave them blank.

Click Next.

 

 

 

This is where you enter all the information about your new hire.

First Name and Last Name fields cannot contain special characters such as spaces, hyphenation or apostrophe.

The Role selection box is visible to some departments. If you see this pick the most appropriate role for your new employee. It helps IT and Facilities to understand what this person will be doing.

Position Type: Permanent or Contract. If permanent you only need to provide the start date. If select contract you will need to provide the end date also.

Location: Pick where the employee will work from, if this person works from home, select Remote Worker.

Employee number: is needed for the Unit4 financial system access. You can get this from Dayforce. If you don’t know just leave it as it is, you can always let us know later.

Phone number: if this person does not have a dedicated phone or extension, just enter your main number for your location.

Manager: enter the full name this person will report to. 

D-Hub (DC): Select one or more roles this person will require. 

OMA (NDT): Select the role this person will need. 

Unit4: If the new hire will need Unit4 access to the financial system turn it on. 

Additional Notes: Use this to send special instructions.

 


 

Terminate an Employee

 

 

 

Step 1: pick the organization, DC stands for Diabetes Canada or if you work for the National Diabetes Trust you want to choose NDT from the list.

Step 3: Click the Terminate Employee button. This will ask you some more questions.

 

 

 

4. Type the departing employee’s name

5. By default, the termination date is set to today’s date. If this is a future termination request, click the calendar icon and pick the date. If you had already terminated the employee leave it to the default date.

6. If you want to delegate access to someone else or yourself type in the person’s full name who should have access to the employees, email, files or voicemails.

7. Click Submit when you are done
