• Go to the Self-Serve Portal at https://services.diabetes.ca/support/home and click on the  "Request New Service" icon.
  • Click on D-Hub under Service Categories
  • click on Request for Event registration link

  • click “Request Now”


  • Complete the request form and click “Place Request” 
  • The request will be assigned to a member of the IT team to provide the service. 

NOTE: You can track the progress of your request by clicking the "Your Tickets" icon in the Self-Serve Portal.
